Our brand new 18,000-square-foot facility located off U.S. Route 224 promotes individualism through self-exploration and community membership. The Village by Gateways features a sensory room, theatre, computer lab, gaming center, a relax and enjoy room, cafeteria, large outdoor patio, green space and more.

Our main campus near downtown Youngstown offers day and vocational habilitation services as well as non-medical transportation.

Community group homes, or an Intermediate Care Facilities (ICFs) deliver real independence for your loved ones in traditional neighborhoods.
Individual Options Waiver
Staff provides support to individuals who reside in independent living settings. Individuals in supported living enjoy a home that offers a higher level of independence and flexible support patterns.
